Criteria for all awards:

  • Nominees must be a resident, attend a school or be involved with a Surrey sport/physical activity organisation in in the Borough of Epsom & Ewell
  • All achievements must have taken place between 1 September 2018 and 31 August 2019
  • Nominees are required to meet a minimum of 2 criteria from the list to be considered for the award (with the exception of the Active School Innovation of the Year which must meet minimum of 1 criteria)
  • There are age criteria for some of the award categories - these are displayed where applicable
  • Categories that have no age restrictions are open to any age. If your nomination is for someone under 18 years old please ensure their parent/guardian is made aware
  • The decision of the independent selection panel is final and will be based on the information provided within this application
  • Before nominating, you MUST have got the permission of your nominee to pass on their details. For data security we cannot handle any nomination for which permission hasn’t been granted.
  • Please read the county level Privacy Policy and the Privacy Policy for your area before nominating
  • Entries must be received no later than midnight on 20 October 2019
  • AFC Ewell Coach, Leader or Instructor of the Year

    This award is for an individual who has:

    1. Made a positive impact by increasing participation and/or inspiring & encouraging others to improve in their sport/activity
    2. Enabled personal or team achievement
    3. Evidence of personal development in the last 12 months
    4. A professional approach with excellent dedication (please include if the coach is a paid or volunteer coach and the number of hours spent coaching/instructing per week)

  • Inclusive Sports Award

    Will be presented to an individual, team or enabler in recognition of their commitment and achievement in their chosen sport in the last 12 months:

    1. Made a significant impact on disability sport and been an inspiration to others
    2. Devoted continuous time and resources to the development of disability sport
    3. Overcome barriers to excel in sport
    4. Been a role model / mentor for disabled people

  • Epsom Gymnastics & Trampoline Club Young Sports Personality of the Year

    Will be presented to an individual (aged 18 years or under at the time of their achievement) who has:

    1. Made significant progress in the field of sport
    2. Had significant impact on sport in Surrey and been an inspiration to others
    3. Exceeded expectations in their sporting performance or made significant difference to a team

  • Surrey Playing Fields Volunteer of the Year

    This award is for an administrator, coach, official, or general helper or a combination of these over the last 12 months who has:

    1. Demonstrated enthusiasm and commitment to a sport or club
    2. Made a tremendous impact on a club or organisation through volunteering
    3. Shown a commitment to personal development, team working and supporting the development of other volunteers

  • Epsom & Ewell Colts Active Community Project of the Year

    This award is for a project that can:

    1. Evidence a sustainable increase in activity levels of Surrey residents
    2. Show innovation to help participants overcome barriers
    3. Have had a broader impact beyond activity levels (physical wellbeing, mental wellbeing, individual development, social/community development, economic development)

  • The Rotary Club of Epsom Outstanding Service to Sport Award

    Will be presented to an individual who has:

    1. Contributed significantly to sport over a minimum of 5 years
    2. Demonstrated continued enthusiasm and devoted time and resources to enable others to take part (please include the range and number of people helped and any personal sacrifices made)
    3. Increased participation and enjoyment of sport and made a positive impact in their local community

  • Better Sports Personality of the Year

    Will be presented to an individual (aged 19 years or above at the time of their achievement) in recognition of their commitment and achievement in their chosen sport in the last 12 months. They will have:

    1. Made significant progress in the field of sport
    2. Had significant impact on sport in Surrey and been an inspiration to others
    3. Exceeded expectations in their sporting performance or made significant difference to a team

  • The Royal Automobile Club Active School Innovation of the Year

    This award will recognise a school (Key Stage 1-2) that has shown exceptional commitment, creativity and innovation to PE, school sport and/or physical activity over the past academic year, enabling pupils, particularly those who need it most, to be more active, more often. Successful schools will have a demonstrated tracked record in delivering at least one of the following:

    1. A creative approach to engaging children and young people, including the less active, in PE, school sport and physical activity.
    2. The integration of physical activity and healthy lifestyles across the whole school day and the promotion of the benefits of daily physical activity.
    3. Using sport and physical activity as a tool to make a positive impact on the health and wellbeing of children and young people (including mental health).
    4. An increase in the proportion of young people from under-represented groups taking part in regular physical activity.
    5. Delivering programmes to encourage young people and their families to be active together
